The Generalised Anxiety Disorder-7 (GAD-7) is a 7-item self-report measure of anxiety severity. Each item is rated 0-3 (not at all to nearly every day), producing a total score of 0-21. It is the IAPT core anxiety measure and is administered at every clinical contact.
Over the last 2 weeks, how often have you been bothered by any of the following problems?
Use as a validated screening and outcome measure at assessment and at regular intervals throughout treatment to track symptom severity. The GAD-7 is recommended by NICE and IAPT as a routine outcome measure for generalised anxiety.
Frame as a brief check-in tool that helps both therapist and client track progress over time. Normalise the process by explaining it is used routinely in evidence-based therapy services.
For clients who find self-report measures anxiety-provoking, complete it collaboratively in session first before asking them to complete it independently. Consider using alongside the PHQ-9 given high GAD-depression comorbidity.
The GAD-7 screens for generalised anxiety specifically and should not be used as the sole measure for other anxiety presentations such as panic disorder or social anxiety, which have their own validated measures.
Look at individual item scores, not just the total, to identify specific symptom clusters. A score reduction that crosses a clinical threshold (e.g., 10 to 7) is clinically meaningful even if the total change seems modest. Use score changes to reinforce client progress and motivation.
Scores are calculated automatically when your client completes this measure (range 0–21).
